Job Summary

The Outreach Case Manager is responsible for outreach and recruitment of the RISE (Resources to Initiate Successful Employment) program clients on and off site. This role is responsible for making in-depth assessment of students' needs and developing employment plans to support students to successfully complete the program and obtain employment based on an individualized employment plan. Responsible for managing case load, referrals, meeting target goals and acts as an advocate/facilitator to help meet student needs.

 

Essential Functions:

On and off-site recruitment of potential RISE program students.

Participate in all RISE program related meetings.

Case manage students to reduce barriers and meet basic needs. Assess needs, give guidance, and monitor progress through internal and external databases.

Provide services and/or referrals for needs through direct services utilizing Goodwill resources, or by referral to another organization, connecting students to outside agencies as needed.

Provide advocacy, and follow up with providers to whom referrals have been made, or that have made a commitment of resources to the student.

Complete intake assessment including all required forms and documents for student's enrollment.

Maintain clear and accurate case notes in databases, and maintain paper and electronic student files.

Build professional relationships with students and site staff that facilitate successful employment outcomes.

Network with community based agencies to stay informed about area services and resources.

Actively participate in staff meetings and other departmental projects.

Demonstrates safe work practices through awareness and observation in support of a safety culture; reports any potential hazards or accidents.

Other duties as assigned.

Minimum Job Requirements:

Education: Bachelor's in social work or related field preferred; Associate's degree or a combination of relevant experience and education in lieu of a degree are acceptable.

Experience: Minimum 2 years social work, human services or case management experience required.

Specific Skills/Knowledge/Licenses Required: Knowledge of local community resources and agencies, as well as government benefit and immigration policies and procedures.

Ability to communicate and work effectively with students from a variety of economic, cultural, and ethnic backgrounds, physical and mental abilities and sexual orientations and those with limited English skills.

Ability to read and interpret documents such as law enforcement reports, government or agency regulations and legal interpretations. Ability to practice confidentiality at all times.

Ability to work independently and has part of a team. Ability to make good decisions without on-site supervision.

Proficient in Microsoft Office applications, with the ability to learn new software as appropriate and necessary to the job.

Bilingual/multilingual capabilities preferred.

Must have valid driver's license and reliable transportation.

 

Essential Physical Abilities:

Lifting and carrying up to 25 pounds; to reach above head level, developed fine motor control (grasp, handle, manipulate objects with fingers; pushing; able to travel across a variety of surfaces (stairs, ramps, uneven terrain).

Working Conditions:

Office environment; the noise level in the work environment is usually moderate. This position requires regular travel, sometimes between locations, off-site meetings and events.
